Physical Therapy is not just for people that have had a severe injury or complicated surgery; it can also help those with mild problems or chronic aches and pains. Physical Therapy as a general rule should be started as soon as possible. Addressing your problem quickly will often result in reducing the overall treatment time required. Your treatment plan will be evidence-based (according to the clinical stage you are presenting). After a careful evaluation, a treatment plan will be developed; any questions related to it answered, and you will be kept abreast of your progress.

At Crescent Physical Therapy, we make it very easy for you to get started by calling 718-545-0999 to make an appointment or for more information. Typically, we can see you within 24 hours of your call, and that call is the first step in getting you on the road to living your life the way you’ve always wanted to, but didn’t think possible. During the call we will ask you for some basic information to get you started.

We perform various treatment regiments, and will customize a program combining the best treatments to resolve your particular injury. Regardless of age or ability, the goal is to design a treatment regiment appropriate to the level of the individual. The physical therapist will solicit your participation in establishing the treatment regiment and goals.

Wear comfortable, loose clothing and athletic shoes. During Physical Therapy sessions, it is important to have access to the injured area for effective treatment. You should wear loose fitting clothing so you can expose the area that we will be examining and treating. For example, if you have a knee problem, it is best to wear shorts. If your problem is with your upper back, shoulders, or your cervical spine, you should wear a t-shirt or tank top.

Your initial visit will last 45 minutes to an hour since the therapist will need to evaluate your condition and provide treatment. Following visits lasts 1 hour depending on your injury and exercise program.

Physical Therapy is 2 to 3 times a week for the next 6 weeks unless otherwise specified by your referring doctor.

  1. Completed Registration Form (LIST OF FORMS)
  2. The prescription and/or referral for care
  3. List of medical history, medications, allergies or other information that may be related to your medical condition such as operative reports, X-ray and/or MRI results.
  4. Current Insurance Card(s)
  5. Picture ID

It depends on your insurance provider and coverage. Some insurances are qualified for Direct Access meaning that they do not require a prescription or referral for the first 10 visits or 30 days after your initial evaluation. To find out if your insurance is eligible for Direct Access, please call us at 718 545 0999.

A patient who needs to cancel an appointment must do so 24 Hours prior to their scheduled appointment time. If you need to cancel or reschedule your appointment, please give us a call. If you are unable to provide 24 hours notice, you will be charged $15. This fee is not covered by insurance, and you are fully responsible for the payment. This policy excludes emergencies. When you call, we will help you reschedule your appointment. Giving us as much notice as possible helps us to better serve you and our other patients.
*Frequently missing appointments delays your recovery and can turn a simple injury into a chronic problem.
